Terms of Service
These Terms of Service (“Terms”) govern your use of the Empowerment Therapy Group, LLC website, online forms, scheduling tools, and related administrative services.
By accessing this website or submitting information through it, you agree to these Terms.
If you become a therapy client, your care may also be governed by additional clinical documents, informed consent forms, telehealth consent forms, payment agreements, privacy notices, and practice policies.
1. About Empowerment Therapy Group, LLC
Empowerment Therapy Group, LLC (“ETG,” “we,” “our,” or “us”) is an Alabama limited liability company providing therapy services through licensed clinicians.
Services may include consultation, intake, assessment, psychotherapy, treatment planning, and related clinical documentation.
ETG does not currently offer coaching services. Nothing on this website should be interpreted as offering coaching, life coaching, wellness coaching, consulting, or non-clinical coaching services.
2. Website Use
This website is provided for general informational and administrative purposes only. You may use the website to learn about ETG, request information, submit inquiries, or access scheduling-related tools.
You agree not to use this website to:
- Submit false, misleading, or unauthorized information
- Attempt to access systems or records without authorization
- Interfere with website security or functionality
- Copy, scrape, reproduce, or misuse website content
- Submit emergency or crisis communications
- Harass, threaten, or abuse ETG staff or clinicians
- Upload malicious code, spam, or harmful content
- Use the website for unlawful or unauthorized purposes
ETG may restrict access to the website or services if misuse occurs.
3. No Emergency Services
ETG does not provide emergency, crisis, on-call, or after-hours services through this website, email, text messaging, scheduling forms, client portal messages, or inquiry forms.
If you are experiencing an emergency, call 911 or go to the nearest emergency room.
If you are experiencing emotional distress, suicidal thoughts, or a mental health crisis, call or text 988 to reach the Suicide & Crisis Lifeline.
Do not use ETG’s website or electronic communication tools for urgent or emergency needs.
4. No Therapist-Client Relationship Through Website Use
Using this website, sending an email, requesting an appointment, or receiving a response from ETG does not automatically create a therapist-client relationship.
A therapist-client relationship begins only after ETG confirms that services are appropriate, the required documentation is completed, informed consent is signed, and a clinician accepts the individual as a client.
ETG may decline to provide services when services are clinically inappropriate, outside provider’s scope, outside the legal jurisdiction, unavailable due to scheduling limitations, or otherwise unsuitable.
5. Therapy Services
Therapy services are provided by licensed clinicians within their professional scope.
Therapy may involve assessment, diagnosis, treatment planning, clinical documentation, and mental health treatment.
Therapy outcomes cannot be guaranteed. Progress may depend on many factors, including participation, therapeutic fit, attendance, clinical needs, outside stressors, continuity of care, and treatment goals.
ETG may recommend referral to another provider, a higher level of care, emergency services, or specialized treatment when clinically appropriate.
6. Eligibility and Location Requirements
Therapy services are available only to clients located in jurisdictions where the treating clinician is legally authorized to practice.
For telehealth sessions, you may be required to confirm your physical location at the time of service.
If you are outside Alabama or outside another jurisdiction where your clinician is authorized to practice, the clinician may be unable to provide therapy services.
7. Telehealth Services
ETG may offer therapy through telehealth platforms when clinically appropriate.
By participating in telehealth, you understand that:
- Technology interruptions may occur
- You are responsible for choosing a private location
- You should not participate while driving or in unsafe conditions
- Your clinician may ask for your physical location and emergency contact
- Telehealth may not be appropriate for all clinical needs
- A higher level of care may be recommended if telehealth is not clinically suitable
Additional telehealth consent forms may apply.
8. Scheduling, Cancellations, and Attendance
Appointments may be scheduled through ETG’s approved scheduling process or platform.
Clients are responsible for attending scheduled sessions on time.
ETG may maintain cancellation, late cancellation, missed appointment, and rescheduling policies in separate practice documents, informed consent forms, payment agreements, or client portal documentation.
Failure to attend sessions consistently may result in discharge, referral, or changes to scheduling availability.
9. Fees, Payments, and Insurance Support
Clients are responsible for all fees, copays, deductibles, self-pay rates, missed appointment fees, or other charges described in ETG’s payment policies or applicable third-party platform agreements.
ETG does not bill insurance directly. If a client wishes to use insurance benefits, ETG may assist the client in accessing services through a third-party platform that manages eligibility verification, claims, billing, reimbursement, or related processes.
ETG does not guarantee:
- Insurance eligibility
- Reimbursement
- Claim approval
- Coverage amount
- Network participation
- Accuracy of third-party billing decisions
- Timeliness of third-party insurance processing
Clients are responsible for reviewing any third-party platform’s terms, privacy policy, payment obligations, and insurance rules.
10. Client Responsibilities
Clients are responsible for:
- Providing accurate contact, health, and payment information
- Updating ETG when information changes
- Participating honestly and respectfully in services
- Attending sessions on time
- Using secure communication methods when available
- Notifying the clinician of emergencies, safety concerns, or major clinical changes
- Following emergency instructions when crisis support is needed
- Complying with practice policies and payment responsibilities
ETG reserves the right to end services, pause services, or provide referrals if a client’s needs exceed the practice’s scope, if safety concerns require higher care, if policies are repeatedly violated, or if continued services are not clinically appropriate.
11. Communication Policy
ETG may communicate by phone, email, text message, secure portal, telehealth platform, or scheduling system.
Electronic communication may be used for administrative purposes, including appointment reminders, scheduling updates, forms, billing reminders, or limited service-related updates.
ETG does not guarantee immediate response to emails, texts, voicemails, or website inquiries.
Electronic communication should not be used for emergencies, urgent clinical issues, or crisis needs.
12. SMS/Text Messaging Terms
If you opt in to receive text messages from ETG, you agree to receive limited administrative messages, such as appointment reminders, scheduling updates, or form reminders.
You may opt out at any time by replying STOP.
Message and data rates may apply.
Text messaging may not be fully secure. Do not send sensitive clinical information by standard text message unless instructed through a secure process.
13. Privacy and Confidentiality
Your use of ETG services is also governed by ETG’s Website Privacy Policy and, if you are a therapy client, ETG’s HIPAA Notice of Privacy Practices.
Therapy information is handled according to applicable confidentiality laws, professional ethics, HIPAA requirements where applicable, Alabama practice standards, and clinical policies.
Confidentiality has legal exceptions, including but not limited to safety risks, mandated reporting, court orders, subpoenas, licensing board inquiries, regulatory investigations, law enforcement obligations, and other circumstances required or permitted by law.
